Outstanding Volunteer Award Nomination Form

Nominating agencies must be VCRI members during the 2007 calendar year to have a volunteer selected. The deadline for Outstanding Volunteer nominations is March 16, 2007.  All materials must be in the VCRI office by March 16, 2007.

Supporting materials are welcomed (newspaper articles, samples of the support the volunteer has given your agency, etc.).  Items must be postmarked by the deadline.
